All participants can register through the website using the Online Registration Form.

Please note that no Registration Form will be accepted unless accompanied by proof of full payment (copy of bank remittance) sent to the Organizing-Administrative Bureau/Secretariat, Conferre Ltd, by email (admin@live2012.gr) or fax (+30 26510 68611).

Companies, who wish to invite participants to LIVE 2012, should contact the Organizing-Administrative Bureau (info@live2012.gr) in order to be informed on the procedure, terms and conditions.
Please note that in case of participants, who wish also to register for Hands-on Practice/Workshops, it must be distinctly mentioned for each one individually, through the correspondence with the Bureau. Registration for the Symposium does not presuppose registration for Hands-on Practice/Workshops.

Notes:

• Please note that companies will benefit from the early registration rates (as mentioned in the table below) provided that there will be full pre-payment of the fee until April 20^th, 2012.
• For any written cancellations before/on April 20th, 2012 there will be full refund. For written cancellations received from April 21st until April 30th, 2012, there will be 50% refund. After this date, cancellation will not be entitled to any refund.
• The list(s) with participants’ names must be strictly provided until April 30th, 2012.

LIVE 2012 course directors taking under consideration the global financial crisis, has decided to reduce the registration fee for trainees in order to encourage their participation.

Please note that from May 19^th, 2012 & onwards registrations can only be made On-site at the Secretariat. Pre-registrations will be accepted, provided that the Registration Form and the corresponding proof of full payment will have been sent to the Organizing-Administrative Bureau no later than May 18^th, 2012.

Nursing staff and students will be requested to display their Identification Card/a relative document, on Secretariat for their registration on-site.

• ONLY doctors & trainees are entitled to participate in the Hands-on Practice/Workshop
• PRE-REGISTRATION is required through completing the appropriate field in the Registration Form. Requests for registration sent by email or fax will not be considered.
• Registration for all workshops is free of charge, however pre-registration is required.
• Registration for the Symposium does not presuppose registration for Hands-on Practice/Workshop.
• Registrations will be honored on a first-come first-served space available basis.
• Registrations for participating in the Hands-on Practice will be accepted until April 30th, 2012 or until the maximum number of participants is reached.

For written cancellations of registration sent to the Organizing-Administrative Bureau

• Before & on 20/04/2012: there will be full refund
• From 21/04/2012 until 30/04/2012: 50% of the amount will be refunded
• From 01/05/2012 & onwards: there will be no refund

All cancellations and/or amendments have to be made in writing to Conferre Ltd.
No telephone cancellations and/or amendments will be accepted.
Refunds will be settled within 30 days from the end date of the Symposium.

In case you wish to issue an invoice, please check it the invoice box and complete the appropriate fields of the registration form with all the necessary details. In case of participants do not specify their request for invoice or receipt Secretariat will issue automatically a receipt.

Receipts and Invoices for Registration expenses will be provided to participants during the Symposium onsite by the Secretariat, upon request. No receipts or invoices will be sent in advance.
